There is no cost to submit an application or to appear in the Foxhole veteran directory. To keep the directory trustworthy for veterans and their families, every program is vetted before it goes live.
- 1Submit your application. Complete this form with your program details, coverage, and specialties.
- 2Credentials review. Our team verifies your license or accreditation, checks your VA / TRICARE / CHAMPVA claims, and confirms you actively serve veterans or active-duty service members.
- 3Contact confirmation. We reach out to your listed contact to confirm the information is accurate and that you can respond to inbound inquiries from veterans and families.
- 4Approval and go-live. Most reviews complete within 5–7 business days. You'll get an email either way — if approved, your free listing goes live immediately.
Optional upgraded placements (Verified and Featured tiers) are available after approval — they are never required to appear in the directory.
What we verify
- State license or national accreditation is current and in good standing.
- VA, TRICARE, or CHAMPVA claims match the payor's directory or a coverage letter.
- Program actively serves veterans, active-duty, National Guard, or Reserves.
- Listed levels of care, specialties, and clinical approaches are accurate.
- Intake contact is reachable by phone and email within one business day.
Documents you'll be asked for
- Current state facility license (or equivalent for outpatient / telehealth).
- Accreditation certificate — JCAHO, CARF, COA, or SAMHSA — if applicable.
- Organizational NPI number (Type 2) for insurance verification.
- Proof of payor relationships you claim: TRICARE contract, VA CCN letter, CHAMPVA acceptance.
- A working intake phone and email — no shared inboxes or personal addresses.
